No one knows what the name will be except JKR. All we can do is speculate.
As for the release, the most commonly rumored date is 07/07/07. JKR is working hardcore on the book this year, and she recently said it's "coming along nicely" or something like that, so an '07 release does seem likely. Also, 7 is a really crucial number in the series, so it makes since to try to make it available on that date.

Only J.K knows, but she has said that two main characters are going to die, and one might be harry.
I think the next title will be: Harry Potter and the Final Duel
...or something along those lines:)
