what is the procedure should follow in order to change name, need to fill some forms?, and where to submit, etc.

You need to bring yoru marriage lic. to the DMV and Social Security Office for all of your credit cards you just need to call them and as simple as that your name is changed. You will need your marriage certificate also to change your name on your bank accounts.

As soon as she signs the marriage contract the woman has THE OPTION to change her name. She doesn't have to.

After that she just goes to each and every agency where her change of status or name is required. Social security, internal revenue, the bank, credit card companies, etc. etc.

There is no law that says a woman IS REQUIRED to change her name.

You take the marriage certificate to the court house and swear your name changed, they will give you an id card and then you take that and the marriage certificate to the DMV and to Social Security and anywhere else you have id.

A girl I worked with changed her first and middle names. It's been so long ago, though, that I can't rermember what it involved. I do know that she had to get statements from two people attesting to the fact that she wasn't doing it to escape anything.

When you filed your Petition for Divorce you attorney should have requested your name change. If not, you District Court Clerk can provide you the form,

Bring your marriage liscense to the DMV and the SSN office. Problem solved.
