When I hear people preface their comments that way I suspect they are testifying against themselves. If they were not "being honest" before then they were lying.

Most often, I feel people use these phrases when they know that there is going to be a difference of opinion on an issue. I have frequently seen people use them before delivering an abrupt or curt comment, or an answer when they want to be fully honest with a person without intentionally trying to hurt the others feelings. For example: You and your significant other go out with another couple, that are your friends. At the end of the evening you ask, "So what did you think of them?". The response could go something like this. "I had a good time, but to be honest with you, I really didn't care for them all that much.

Sometimes the truth hurts, so they are just preparing you for some brutal honesty

In my experience, people also say "to be honest with you" when they are about to say something tactless and rude!
