There have been several Timeladies on the show, and one was a regular companion in the late 70s/early 80s. Her name was Romana, and she had one regeneration (so two incarnations). She was originally played by Mary Tamm, and regenerated into the body of Lalla Ward, (who then became Doctor Tom Baker's wife in real life). Romana ended up bogging off into an "extra" universe, E-Space.
Plus there's considerable agreement that the Doctor's first companion, Susan, was genuinely his grand-daughter, as she always claimed, which would make her the first Timelady to appear on the show.
In the revamped series, they're making a big deal that the Doctor is the "last of the timelords", so the chances of a timelady making another appearance at this point are slim, but you never quite know with Russell T Davies.
As to the question of whether the Doctor should regenerate into a female body...technically, nothing wrong with that idea. I think the moment might have passed though - there was a lot of talk about it years ago (and to be really pedantic, Joanna Lumley briefly played the Doctor in the Comic Relief spoof)...but now he's technically only got three incarnations left, and ther'd have to be a strong argument that the female incarnation could a) be around for quite a while, and b) bring real dimensions of new material or experience to the character and the show. Might work, but I don't know if they're too near the end for this now...

Not sure if that would work, I don't think people would warm to a female Doctor, and lets face it Dr Who has been going for 43 years so I think the writers know best by now, and if its not broken, don't fix it - these last couple of series have been the best ever

Only if it were a particularly hot regeneration, wearing as little underwear as you can get away with for a teatime audience!!!!!!!! Seriously, it would take a very special actress to break down the sexual stereotypes and make it believable. As he/she is an alien anyway, I don't see why not technically, but you're trying to mess with 2 generations of childhood memories here. A potential landmine of Biblical proportions.
