Her vagina is not sealed. There is a thin layer of skin called the hymen which can stretch or break prior to intercourse. So there is a large enough opening for menstruation and thus, a tampon.

Virgin girls have a thin screen termed hymen, but with a small puncture to allow menstrual blood flow out. Often, during manual examination, or due to games etc, hymen gets torn. in that case, giguratively, virginity is lost.
Otherwise, hymen punctures and ruptures during first sex, if the partner is forceful enough. ON the other hand, if girl experiences pain and he withdraws, virginity is preserved. If Hymen is strong, you may need surgical rupturing to enable sex.
