i'm not talking about stress or heat, but what actually happens in there that makes it hurt?

Have you ever had one of those really bad headaches where you can feel your heartbeat in your head? Headaches are caused mostly by constriction of the blood vessels in and around your brain. They can be mildly painful to excruciating.

The fluid pressure in cranium increases.In fevers it is the hot running blood,in colds cranial openings blocked by nasal secretions,in migraines it is the dilated temporal artery,and in tumors it is the blocked pathways of cerebrospinal fluid and so on.

Dehydration is one known cause but there may be others. Like a brain tumor maybe?

Stress or other health problems. If they are really bad and happen frequently check with your doctor.

Migraine. If not, try to brew some ginger and drink it
