A graph is usually a visual representation of a function.

A chart is not a mathematically defined entity. It doesn't have to represent a function or even something mathematical. It just conveys some sort of information that may be some sort of data.

A chart is a simple drawing of a box with data and numbers to relate information.

A Graph would show bars, or pie chart or pictures to show how each data is in relation to another.
