Go to the empty end of a parking lot and practice. Sounds like you just need to give it a little more gas, but your clutch could be warn as well. Its all about getting just the right touch.
Like I said find an empty parking lot to practice in. Try just landing between the lines. Once you get comfortable, try finding one with some curbs or median type structures. Try paralleling to the curb. Then maybe try bringing a friend with a beat up car along so you can park between it and the curb. Get creative and be careful.

Take your car to a parking lot of some business that is closed and put it in the middle of the lot. Put your car in reverse, slowly release the clutch-- with your right foot off of the brake and not on the gas. If the car starts to die, push in the clutch and then again slowly let up on the clutch as you slowly give the car a bit of gas. Once the car starts going backward, you can practice adjusting the gas level. You can concentrate on the clutch/gas situation without stress in that big empty parking lot until you have it down.

The main thing is to put your handbrake on. Put your foot on the clutch and put it in reverse. While your foot is still on the clutch, give it a little gas and release the clutch half way until you feel the 'friction point' this is when you feel the car tugging its way up. When you feel the friction point, release the handbrake and your acceleration should not bring your car to a halt.
Once you get use to this procedure, you should have no problem doing it with the help of the handbrake.
This same goes with uphill but make sure you don't put it in reverse ;)

You use the clutch and brake only. Hold the brake and shift into R. Ride the clutch and fluter the pedal till it catches the gears . NO GAS. The motion of the letting up the clutch a few times with out stalling should coast you into the spot. Gas will move you too fast & you'll hit something.

The trick really is to not think about it...and it will become natural. Think of the clutch and the gas as a teeter totter...they both can't be in the same place..just life your foot off the clutch slow and smooth while you press on the gas slow and smooth...think of it all in one fluid motion. It really is about getting over the fear of stalling...once you do that, you'll be golden. Good luck!
