I have been offered $5,000 with a monthly periodic rate of 2.916% and a 34.990% Annual percentage rate. I know this is not good but just how bad is it? Would I be absolutely out of my mind to take it?

Well I also just got an offer for 0% APR for one year on balance trasfers. If I am sure to pay it off before then, that is a smart move right? It would be saving money to transfer from a couple of accounts that have between 18 and 24% right?

34.990% is ridiculous - who monitors those companies - I would report them becuz that is highway robbery
