A fetish is an association of an inanimate object with sexual pleasure. For instance one may have a shoe fetish or a leather fetish. Sometimes in the media people will talk about foot fetishes. This does not meet the clinical definition of a fetish. It is a pop culture term because a foot is not an inanimate object. It is important to note that even though corpses are inanimate a person with Necrophilia is not classified as having a fetish but is classified as having another disorder entirely. A person develops a fetish by association so if one has sexual experiences with another individual or individuals who wear a specific item before sex one may begin overtime to associate that particular item with the pleasure he/she receives from sex.

Boredom. Neanderthals didn't look for different sexual positions...they were driven to reproduce. We don't really need to reproduce anymore,we're over populated. Some animals when over-populated commit suicide in an attempt to save food.Us...we create diseases. Why not go out with a weird fetish and come home with HIV.Keep it up,you are saving the rest of us!!

Childhood sexual trauma (real or imagined), genetics, a physiological issue, experienced dissassociation, modeling from early childhood or a combination of these.

It depends on the severity, and what exactly is going on.

From little things that happen when we're younger and then things that excite us when we're older and we choose to act on them. Anything that we choose to dwell on and let excite us can become a fetish.
