Im not sure how true this is but i did read some where that they did a few tests with the Hulk naked, probably using camera angles and lighting to hide his junk.
But that would mean missing out on lots of action shots to hide it, so the stretch-pants theory comes up.
Also the comics began in a really repressed era sexually.
