just don't press the clutch when your driving or coming to a stop as much (or when your shifting) so try to shifting quickly but smoothly while shifting into neutral when u come to a stop

Riding the clutch means that your foot is still on the clutch, causing pressure on the clutch plate. It will wear down your clutch very fast and its a habit that you want to get out of right away.

usually it means your resting your left foot on the clutch pedal while you drive. it also means your using the clutch much more than you should (e.i. pumping clutch pedal to allow engine rpm's to rise and boost car along). you may find different explanations based on certain territories as well.
