It depends on the manufacturer. The one I worked for years ago made it in pre-formed pieces (specifically, the ones that protect your computer, that you pull out of the box stuck to either side of the CPU tower), and then simply wrapped bundles of that in shrink-wrap, and shipped it like that.

Stryofoam isn't fragile, so I imagine they use a box without any sort of padding.
