hammer on - hit the fret and hold finger in place (hard enough to make a tone)
pull off - remove finger from fret and let the string vibrate (simultaneously removing fro the fret and stroking the string)

You put your fretting finger down hard so it plays a note without using your pick or picking fingers. Pull off you pull off your fretting finger and let the new note ring, either from a finger which is behind the fretting finger or from an open string. You can see what this is by putting your first finger on any fret and keeping it their. You then repeatedly tap with your second, third or fourth fingers on the same string but higher up to create the hammer and pull off sound.
This works best with an electric guitar.

A pull-off can be thought of as the opposite of a hammer-on. There are 2 notes involved. Before starting, you will need to have both left hand fingers that are involved already placed in their perspective frets. The first note is plucked, then a second note is sounded by pulling that finger off of the string with force. You are basically plucking the string with the left hand finger that you used for the 1st note first note. You will need to pull both towards the floor, and out away from the neck of the guitar.

ok its easy... hammer on we'll start like this
hit an open string then without re-picking play the next fret... u "hammer" your finger on the neck
and a pull off
fret a note and pull ur finger off the string causing it to ring open or ring on a lower fretted note you are "pulling" your finger off the string

litraly what you do with your finger
from the say you are playing a note on the 3rd fret and you pluck it and u let your finger off the 3th n ul have another waiting on the 2nd - thas pulling off - hammering is the oposite direction where you will move up the string once u pluck from the original note - you finger litraly hammers onto a fret above it
so you could do 3p2h3

It's when you use your left hand fingers (or the hand you fret the notes with) to either pluck or hammer down on to the string... it is the ability to sound notes without picking the string with your right hand.

hammer on - you strike the string with your left hand at the fret to make it sound, without using your right hand or pick.
pull off - basically the opposite. you pull your finger off the fret with enough force to make the string sound, without using your right hand.
