Around 20 weeks they may do an ultrasound and that time they can be pretty accurate in telling the sex of the baby.
Some women start to notice an increase in breast size right away. It stays on course with your overall weight gain usually but as you fill with milk the last month they may get quite big. You won't really lactate until the baby is born. At that exact moment your milk is ready and you will begin producing milk for the baby or go through the leaking and drying up stage if you choose not to nurse.
Of course these are all generalizations and each woman is different as is each pregnancy.

Usually around 16-20 weeks u can go to the Dr and they will send you for a sonogram then they can tell you the sex. IF the baby allows it, lol. They move around a lot and sometimes it's hard to see that early. You may have to wait if you have an active baby.
Colostrum is building up throughout the pregnancy and when you deliver, you have colostrum with which to feed the baby. Milk comes in a week or so later.
Breast increasements depend on the woman. No telling when or how much bigger yours will get.
Check out babycenter.com. it's a great site that takes you thru each week of pregnancy and beyond.

20 Weeks
Your baby is now big enough that you can see if it's a boy or a girl on an ultrasound. It can suck a thumb, yawn, stretch, and make faces. Soon -- if you haven't already -- you'll feel your baby move, which is called "quickening."
You boobs will defently start getting bigger when your milk comes in. I would say everyone is different.
A women can lactate I would say in the 3rd trimester of pregnancy. Been 5 yrs sorry about this answer.

I was 18 weeks when I had my sonogram and found out I was having a girl, my boobs never got much bigger until my milk came in, it came in about 4 days after she was born- she was 3 1/2 weeks early, my body wasn't ready to produce milk yet
my sister always started lactating before she delivered of course her kids were born much closer to their due dates

Usually, it takes 18-20 weeks before you can determine the sex of the baby. No idea on the other questions. My wife had large breasts before she got pregnant.
