Only the most relevent and longest positions you have for the last 10 years. Any gaps can be explained in your cover letter as "worked outside of my profession."

All of them if it doesn't make your resume too long. You definitely don't want to show any gaps in employment. I've had to just list my first couple of jobs and not give descriptions so that my resume will fit on 2 pages.

I have omitted listing one job because I was only there 6 months and then the company folded.

I would say only the jobs from the recent past 3 or 4 years depending on how long you were at each, you shouldn't put too many jobs on a resume, it makes you look like you hopped from job to job.

No more than 5 and don't go back more than 10 years.

id say 3 or 4 jobs relevant to the position or jobs that showcase what special skills you can offer.
