1990 | WOUNDED PRIDE In a 1990 qualifying match in Rio de Janeiro, Chile needed a victory against Brazil to remain in the tournament. In the 67th minute at famed Maracana Stadium, a fan threw a firework missile onto the field.

Chilean goalkeeper Roberto Rojas fell to the pitch with an apparent head injury. He was carried off the field and Chilean players refused to return, claiming conditions were unsafe. Chile hoped to earn a victory because of fan interference. But after officials reviewed the video, they ruled that Rojas had faked the injury, and awarded Brazil a 2-0 victory.

Rojas was banned for life; Chile was barred from the 1994 Cup.

'We were yelling in front of the TV at the injustice of it at first,' Chilean FIFA referee Osvaldo Talamilla Lobe told the Mercury News in an interview conducted in Spanish. 'Then we saw the replay and felt the shame in our hearts. Our great Roberto Rojas had lied.'

I haven't heard of any team being banned.
