There is a very strong possibility that this answer could be wrong!

High usage of the HD indicates that some programs are running on your computer. Even if you are not doing anything on the machine, programs could be running in the background.

Check your task manager to see what is active.

If you are running winXP then check services.

You may also be using too much memory. Again check task manager. An over active HD could be page swapping to use virtual memory.
You may need to buy more memory.

Despite what another answer says, this is NOT normal behavior for a PC

A PC not connected to the internet with no applications running and 256 MB of ram, no screensaver will NOT randomly access the HD after a few minutes.

Probable suspect: Virus scanner doing a full check of the computer

Probable Suspect: Adware, spyware

Probable Suspect: Log file writing, a device is coming on or off and the event file is being updated

Probable Suspect: Norton Utilities. For some reason it cant resist making the hard drive go.

Probable Suspect: Active Desktop

Possible Suspects: Virus, windows updates, real media, ect.

Possible Suspect: Your machine may have a very low amount of ram, however usualy this only magifies the problem, it doesnt create it.

Solution (windows) : Control Panel -> Display -> Screen Saver -> Power (button on page)

Reason: Keeping the HDD on all the time saves your computer from lagging, this is because it takes time to spin up the HDD to read from it so it is not good turning it off. However, if you are on a laptop (notebook, oragami whatver) you will probably need this to save battery power.

If no background process is running (ex. any server), then your pc might got hacked too! Firstly unplug your modem if you are using internet and then perform virus/spyware/adware scan using any good software tool.
Try this online tool, but you need axtivex enabled and MS IE to perform scan.
