The book that I have and like:

UML Distilled: A Brief Guide to the Standard Object Modeling Language, Third Edition, by Martin Fowler

It tells you just what you need, with a view toward using the UML diagrams in realistic scenarios. It is not overly academic and it concentrates on the diagrams that real programmers use most.
