What an interesting question! I suppose it's because there's no trust that everyone will be on their best behavior. I also think that it's a taboo-like guard against pedophilia.

Where I live (Vegas) there are. There are restrooms labeled "Family" or there are restrooms with a male/female symbol that you can go into separate or together. Most of the people here do not really care anyway...It's kinda a free for all. The movie theaters and clubs are real bad...lol.

Some places in Europe there are unisex restrooms. There are also unisex restrooms at some clothing optional parks (nudist resorts) around the US.

Well, I guess people figure they might get used as something other than bathrooms if multiple people of both sexes were in there at the same time. At least, that's the obvious reason.
