1) Is the GUI easy to look at or cluttered?
2) Are the applications user friendly?
3) How many problems have you encountered, such as javascript errors or paging errors?
4) When data was returned, was it very slow or did you retrieve data quickly?
5) Do you think our application needs user training?
6) What improvements would you make on our website?
7) Did you get the information you needed from our site?
8) What additions would you make to the site?
9) Overall, what did you think about the website so far?
10) Would you return to our website in the future?
