I think that would go like this...Do you agree with this statement?

No...it should be

"Do you agree with this statement"
or
"Are you in agreement with this statement"

It should be, "Do you agree with this statement?"

Alternatively, I suppose you could ask, "Are you in agreement with this statement?"
