It means that sometime in the last 20 mins. they have attacked another player in the wilderness.
If they die during those 20 mins., they lose all of their items, instead of keeping 3 of them.

Theey are players that just left the wild and have been fighting other players the skull and cross bones last about 5 min

If/ when they die, they'll lose everything, instead of keeping items, as they've just killed another player.
