first dislocation, collapsed on mound; after rehab, returned to game; first game back, injury occurred again in the midst of a pitch. I think he is right-handed.

If you mean Dave Dravecky of the SF Giants, he didn't dislocate his elbow, he broke his arm., not once, but twice. During his second game back after a year off (1988) to rehab his left arm. he snapped the humerus bone and then again during the Giants 1989 pennant winning celebration. He later had to have his left arm amputated.

Dave Dravecky is the answer, but let me clarify the cancer situation.

They discovered a cancerous tumor in his pitching arm in 1987 and managed to surgically remove the cancerous cells. He made a comeback in 1989. In his second game back, his humerus bone snapped. Cancer cells were found to have returned to the arm, subsequent treatments were ineffective, and his pitching arm was amputated.

Truly a sad, sad situtation for a helluva SF Giants pitcher.
