Internally, it will be red and irritated and produce lots of stomach acid and mucous...just like a person who has a stomach ulcer. On the outside his stomach may or may not be bloated.

an ulcer is a whole in the stomach lining. This lets the dangerous stomach acid into the rest of the body and it eats away at the intenstinal muscles and the stomach muscles and causes severs pain. a lot of horses can get them from the wrong kind of hay, or just eating one type for too long, it gets built up at the boittom of the stomach and plugs it up so the acid sits in the stomach for too long and burns a whole through the wall. most of the time they are visibly undetectable from the outside, but you can get a vet to use a camera to look inside the horses stomach.

The only way you could tell is with an endoscoptic camera.
From the outside there would be no symptoms.
Some horses crib and have small cloic episodes and do not hold weight well with ulcers.
If you show alot or your horse is training hard like more than 3 times per week and you trailer to shows or your are racing your hosre you he is a good canidate for ulcers.
They make several things to treat them and help prevent them.
Call your vet and see what he/she thinks about treatment for your horse.

Gastric ulcers are only seen on endoscopy - placing a small camera down into the stomach, and looking around. Only veterinarians can do this, and sometimes only at a specialty clinic. If you suspect that your horse has ulcers, contact your veterinarian.
Some of the signs may include decreased appetite, signs of colic, etc.

Assuming you mean "ulcer", you can't tell from the outside until permanent damage has been done. The stomach lining on the inside begins to bleed, and can ordinarily only be diagnosed by a veterinarian.
