The best place to start is to look up thier address. Send them a letter telling them what you would like to do and seek thier permission. Be sure to keep coppies of both your letter and their letter as well.

Send them a letter asking for use of the photo and which photo it is that's the idea of 'Expressed Written Consent'

Send them a letter requesting the permission, with the details of exactly what you want to use and how you want to use it.
