Not much info to go on, but here's a couple of guesses.
When you say weak, I will assume that you refer to the ability of the heart to pump (such as ejection fraction). There are a number of things that can affect this ability to pump. In america the number one cause is coronary artery disease, decreasing the blood supply to the heart.
Cardiomyopathy, a general term that refers to a problem with the muscle itself can be caused by a viral infection, ischaemia, or the cause can be unknown. In any case the heart again doesn't pump well. As cardiomyopathy worsens the ability of the heart to pump decreases leading to weakness and inability to do any exertion. In severe cases, they require transplant

Usually it is a long term high blood pressure condition. This may be itself caused by many things. Obesity, not enough exercise, wrong diet, etc. You had better tell him to have a medical checkup and then the doctor will tell you the real answer. It is impossible to diagnose someone without seeing him.

The heart is a pump ... a mechanism for pumping blood around the body. as long as we are alife, it is pumping blood, whether we are asleep or awake. human beigs, like all creations are not perfect systems. our hearts, like all pumps can fail suddenly like our car pumps do all the time. the pump of a new car fail, so can the blood pump we call heart. diseases that weaken heart muscles or overload them with work, like hypertension, make them fail.

we have two types of heart. physical and mental! if you mean the first, i can say hypertension and angina/infarction are the two common causes responsible for some 70% of cases with weak hearts.
