You have to make sure your insurance provider knows. And re-register your kid under her new name. Just go to the court offices downtown in your city (That's where they are in mine) and get the proper papers. It's quite a bit of paper work. Fill out the reasons, the new name (full name) and you have to obtain the signatures of the other parent or guardian.

I don't know about now but when my son was little I was able to change his last name at the birth certificate office because his dad's name wasn't on the birth certificate. If her dad's name is on there then the best advice is to call a lawyer that offers a free consultation and ask one of them. Good Luck!
