There is indeed one in development that a researcher used on himself to test out embedded technologies. There are number of news articles about it (well not really news since it was 1998)

Yes, there are several kinds of microchips, designed to be embedded in people.

For instance, most peacemakers that are implanted under the heart patient's skin use some kind of microchip.

Also, there's a club in Spain, the customers of which can get an RFID chip implanted under their skin. The chip is used as a pre-paid phone card and allows them access without them having to worry about paying with money or credit card at the entrance. This has been going on for a couple of years already.

Yes there is, it has not been installed in everyone but it certainly has been installed in Pets and there are some people it's been installed in.

For instance there is a Tracking chip in His Royal Highness Prince William so that the authorities can always know where he is.

they have one already made but it is under testing and they aren't sure if they would be able to use it on people with all the laws that are around.
