There should have been a leaflet explaining. It's DVLA Swansea, but I don't know the postcode.

If you are in the UK, it goes to the DVLA in Swansea.

There is a booklet which goes with the application form, and this will have all details - how much it costs, where to send it, how long it will take to get your liscence etc..

Depends what country you are in, must be the states, they dont know there are other countries?
Try the post office, or turn the form over and look at the address to send it to section.

If you are talking about a drivers licence, and you have never had one, then you need to go to the nearest DMV, Department of Motor Vehicles, and take a test.
