Be honest and considerate of the others feelings at the same time. Honesty is always the best road to take. But if it's something like her breasts are to small or she has ugly feet, obviously telling her that would be hurtful, so just say something that is honest with how you feel but doesn't actually reveal something hurtful for her. Being honest and caring at the same time will help you end things on a good note.

Before saying good-bye, make sure that no explaination from the other side will entice you to change your mind.
Then be honest, just go and tell.
If it is 'corporate' good-bye, and has complications then put another question.
