As long as your babys father comes with you to the registry office then your child can have his fathers name. This will give his father parental responsibility for your child.

Yes! However it is much easier to do this when the birth is registered - and I think his father has to do this, probably with you there.

No he has to sign the birth certificate and give permission for you to use his name... at least in Florida
