i also wasnt sure if there is a certain age you have to be before you can set up an appointment.

If you been to that doctor before you should be at least sixteen to set up the appointment, but a parent has to go with you until you are 18 (thats how it is in New Jersey). The receptionists asks if you been there before if you have she will ask your name to pull up your file she will ask what service you need than she will give you a date and time. If you never been to that doctor before they will need to know what health insurance you have, your name, birthdate, last doctor you been to they may need to records transferred over. then she will give you a date and time.

You have a number of pricvay rights under the federal HIPAA law, so generally speaking what you tell the doctor is confidential. You will be asked questions in 3 areas:

1. What is wrong, and how urgent is it?

2. What is your medical history that may assist in diagnosing and treating the issuse?

3. How will you be paying?

on the phone, probably just your name address, phone number, that kind of stuff.... and appt time and what's wrong
then when you get to the doctor's you'll have to fill out insurance information and possibly give your personal info again
