Not necessarily evaporation of sweat. It is the removal of ambient heat being put off by your body. On a very cold, but not windy day, you can stand outside without a shirt on and feel somewhat comfortable. The heat of your body is radiating into the air around you and staying there. If the wind is blowing, then it is taking heat out of your body faster and making you feel cold.

Because the air moving over your skin removes body heat. The more air that passes over, the more body heat you lose. Especially if the skin is wet. That's why we sweat - it's a cooling mechanism. Evaporation leads to cooling. So even if the air moving over your skin is hot (like on a really hot day) - if it's hot, chances are you're sweating, and that will lead to cooling.

Fast air feels cool because of the way your body is. We are warm-blooded so our bodies make their own heat and give off heat as well. Like when your under blankets, your body gives of heat and it bounces back and forth between your body and the blankets to keep you warm, but when the air is moving quickly, and your body tries to heat up the air around it, your body keeps giving off heat and the moving air takes the heat away so your body cools down.

It does, but due to its velocity it also has the ability to carry off heat faster, and that provides the sensation of coolness.

I've heard that in New Mexico, where it is not humid at all, that a breeze feels like a hot hair dryer blowing on you. East coast where it is humid, a breeze is quite a relief.
