First off, I would suggest finding a film or TV production company in your local area. Better to be a big fish in a smaller moviemaking pond like Capetown than to be just another guppy in Hollywood. Star in as much as you can whether its TV radio, movies, telethons, personal appearances, etc. Get together a resume of solid work and start sending the resumes and a clips reel of stuff youve done to talent agents in larger markets. Work your way up, it takes a long time, but if you have talent, talent will not be denied.

Talent, luck, and being in the right place at the right time. Start with local theatre groups. Take bit parts to learn the trade at no pay. Learn how to be different from all the other aspiring actors. Start auditioning for commercials, print ads, live performances, etc. Once a professional resume is built, go to where the business is; New York, Las Vegas, or Los Angeles. Once in town, you can join the hundreds of actors trying to get the same job.

Several ways. You can sign up with Agencies that place extras in films. Those are fairly common, and you get credit as an extra toward becoming a member of the Screen Actor's Guild (SAG). You can also do student films, as you get acting experience as well as working with folks who may be a connection in the future. And then of course, there's knowing people like casting directors, Prodcuers, etc. As much as what you know, is who you know.
