The blue colour of a smurf can be explained by it's circulatory system, which instead of a iron-based issue (hence the red colour) in Humans, the smurf's blood is made up on Copper-Based Heamoglobin.
The Skin of a Smurf, will in fact turn the same colour as a Human who is being 'throttled'. Therefore, this would be a grey/white colour.

Anyway, chances are a smurf would probably just turn a darker shade of blue, since it's not getting any oxygen.
