Very simply explained it means that a person feels in between the sexes.
Transsexual means that a person feels like a person from the other sex, a transgender feels more in between. This person can feel the need to dress like the other gender, but that is not necessarily the case.
I am transgender myself. I have always felt androgynous, in the middle of man and woman. Sometimes more man, sometimes more woman. I know this is not easy to comprehend if you don't feel this yourself.
This is how I like to explain it:
Imagine this. There comes a good witch and you can change sex for one day. Most people will do that out of curiosity.
So for one day you are in the body of the other gender.
Then when the witch comes back, she get's a heart attack and dies. So, you will have to stay in that body for your whole life.
You might get used to this body, and you will start to react on the hormones that dominate that gender. You might even love the body because it is a nice and healthy one. But, inside of you, you will never ever be able to really feel like that gender.
This is a bit how I can explain the feeling. I love my body in the way that it is healthy and, say it myself, a beautiful body. But actually I have never felt 'woman'. Always in between.

Um, people seem to have some MAJOR issues with definitions, particularly transgender as opposed to transsexual. Transgender is an umbrella term referring to people who don't fit society's gender expectations. It's most often used to refer to people who identify as the opposite gender of their sex, but not always.

Uh actually transgendered is an umbrella term for a host of people from crossdressers to transexuals. Crossdressers are people generaaly men who occasionally dress in feame clothing and present themselves as women.
Transexuals are people men and women who feel that they were born in the wrong body and often take steps, taking hormones and having surgery to change their outer appearance to reflect how they feel inside.
Most people think of male to females MtoF when they talk about transexuals but stats show that MtoF and FtoM transexuals are almost equal in number.

TransGENDERED means you are one gender mentally and a different gender biologically. TransSEXUAL means you have takes steps to make the two match. For instance: a biological female who identifies as male is transgendered. That person may dress like a man, take a man's name, live as a man. When that person begins taking testosterone, having surgeries, etc, that person is then considered transsexual.

Not every baby is born Male. Or Female. Some babies have some of the characteristics of both.
Sometimes without surgery, the baby growing through childhood to young adulthood changes in his sexual orientation too. This ios transgendered.

Ugh! I hate it when clueless people answer these questions. The first three people got it wrong. Sangre y Leche got it right and so did the person below him (DKlyde). Transsexuals are transgendered, but not all transgendered people are transsexuals.
