I live in Philadelphia and I would say it's very accessible. I know a few years ago they changed alot if not all of the sidewalk corners so they can be accessed by a wheelchair. Philadelphia also has a service provided by SEPTA the local public transportation company for people with wheelchairs.

The very public tourist places should be accessible. But you have to keep in mind it's an old old city and crowded, so there could be problems.
