No, it's not necessary to be completely nude, but a lot of couples do that because they feel more comfortable, close, and naturally loving together.

No, all that is necessary is that the mans thing gets into the girls thing. You can still have clothes on and this can happen. Also you can get a girl pregnant by having sex, wth your clothes still on.
