Do you want a clinical definition or do you want an esoteric ideal? Websters defines truth as "The state of being the case: fact, the body of real things, events and fact: actuality"; whereas hope is defined as "A desire acompanied by an expectation of or belief in fulfillment"
The definition notwithstanding, it is my belief that there is no such a thing as truth. At least not that we can agree on. There are scientific truths that as a whole we accept as true, but they are true on this world, or in this galaxy, and may not apply everywhere in the universe. Things like gravity, or that every action has a reaction, and so forth. It has been said that there are no absolute truths, and that that is the absolute truth, so this is a great question to ponder metephorically.
If I think about perception when I ponder truth I have to laugh. Each of us has our own perception of what is true and real in the world around us. If I told you that there was a rattlesnake under the chair you were sitting on and you perceived it as real, the fear wouldn't be any more or less real to YOU than if the rattlesnake I thought I saw turned out to be a garden hose now would it. That's a good example of how our perceptions shape our reality. You may have never looked under the chair at all, but you would truly react as if it was really there if you percieved that I was being truthful with you.
Hope is an emotion where truth is a cognitive perception. Hope is a feeling in your heart and truth is an idea in your mind? Is this even close to what you were looking for in an answer?

Truth is confidence and Truth is different for each person based upon perception of Truth and knowledge. When you are sure about your facts, you state them with confidence! Hope is wishy-washy! I don't know if it is True, yet I don't look to gather knowledge of whether it is True or not! I hope(it is or it isn't) true. If you don't search for yourself, then how will you find your TRUTH! Are you just going to be wishy-washy all your life and some people never can make a decision, one way or the other. You can't stand in the middle of the road without getting run over, by falsehood or Truth.

Hope: expectation, anticipation, wish
hope can turn into truth ... sumthin you hope can always come true..
