Well if you got into an auto accident then that should ought to be a hell a good reason... Get the police report and show them that ...!!! That is all the proof they should need !! Hell I tell you now a days Its hard to keep anything . No one is ever reasonable . Gurl if you got 2 make one up then do it but be prepared if they try to call them. Or be forward with them that you cant afford to see a doc. and just show the report of accident... If not start looking for another job, Good luck.

call your doctor. they usually will write the note without you coming in. And if it means an hour out of your day to visit the doctor to get a note to keep your job then whats the big deal? It seems the job is very important to you, so go to the doctor to get your note.

I always make my own 'forms'.. just make something that has a false letterhead of some false doc office or something- then put things at the top like : 'Patient's Name' 'DOB', Id #, SS#, Address, etc..
Then make the body of the document be something like this:
"Nature of visit"
and put in something like:
"Patient was seen due to car accident. Complains of nexk and shoulder pain. Referred to Ortho for follow up, given ibuprohen for swelling/pain.
add a date and a signature..

Thats tough because its acually a felony. I dont think I would risk it. Just go to the Doctor for something else. Tell them you couldnt sleep the past few nights because of allergies. When you check out tell them you need a note.

Do you have evidence of the police report? If you do, your doctor will probably write a note for you. Did you call in sick, or did you tell them that your doctor told you to stay home?

Call the doctors office and request a medical release letter from him/her stating your present condition and your ability to return to work. They usually have no problem complying with such requests.
