In California there are cactus pruning services, check your phone book even if you aren't in Cali. There are tons of varieties of cactus and pruning may be different for blooming ones than, say, tall ones so call the experts.
Most pruning is the same across the board, though. Here's how I'd do it if it were a tall cactus:
Buy some cactus/succulent pruning tools. Cut the extremity off where it connects to the trunk, angling the cut so that it is flush with the trunk. Be careful to not cut into the trunk. Check the cut once a day for infestation until the plant heals itself. DO NOT WEAR GARDENING GLOVES when you touch the plant, the spikes will just get inbedded in your gloves and you'll have to throw them away. I would wear latex gloves and use tongs or something similar to grip the branch.

Very carefully...before cutting, call a nursery and find out if they would like to have it "free for the taking"...they might would love to save it and sell it.
