To be proactive is to act before a situation becomes a source of confrontation or crisis.

This means you are looking into the future and looking for obstacles and taking care of them before they become big issues.

Simply, it means getting off your butt and doing something constructive about a given situation.

A lot of people I work with spend most of their time in meetings, planning, thinking about a project, talking about a project, etc....but the project never gets started or done because they're too busy planning! (See Dilbert.)
