Yes, due to HIPAA you have to sign auth forms. (Even though in some states it was law before HIPAA)
Depending on the MD office, you may be able to do this by a handwrittne letter, or tehy would have a form you can sign and possibly fax

Not a permission slip. Give your son power of attorney. They will keep him well informed and in the event that you are not able to make sound decisions for yourself, your son can make them for you. Just be sure your son undestands your wishes.

the Dr's office should have the paper because i have had to do one a few times, if they say the don't then you can write out something out saying that he can that he can have the information and have it notarized the Dr should accept it
