I a bit worry about being a tiny Thai nanny in the U.S. How do I prepare myself? What sould I care about?

I am not sure if you have already found a family or you are still looking.
My best advice is to make sure you are legal: an au-pair work permit can be secured (the family pays for that, it is expensive) only through the agency (strange but true). DON'T accept to work illegally: not only you would be at risk should any health issue come up (no health insurance!!!), and you could be sent home at any time, but you could be in a very difficult situation if the family is not nice to you.

Second, if you have found a family you are happy about, read as much as possible about the area where you are moving to, so you know what to expect: the climate, the lifestyle will be different. Write to your family, and get to know them: I am sure they will appreciate your interest and care.

Third, it is not only for "rich families", and there are a lot of good kids around - and good families!

I think you should read about American culture. It is a lot different than what you are used to I will guarantee that. See if you can find some typical family profiles online. And I would look for some that are from wealthy families, upper class, and middle income.

Be ready for some bad *** kids. you will probably be going to a rich household where the kids run the house because the parents buy their kids happiness. I wish you all the luck in the world.
