I've done it with an extension pole and a very thin roller or pad, but the toilet needs to be far away from the wall to accomodate the roller or pad with some clearance. I've also turned off the water and disassembled the tank from the base, sitting backward on the commode painted behind the toilet with a thin roller. A hint, bend both water supply lines away from the wall and let the plates dangle from the supply lines.
Worse case scenario, the one I just finished! It is a one piece commode that I had to wedge between the wall and the commode, on my side, on each side of the toilet. In that case, make certain the toilet has been disinfected on the outside and on the floor. It was the pits, too because it was a 2 step faux finish.

Take the top off the tank. Tape off around the edges of the tank. Paint under the tank down to the base boards and around the tank with a brush. You will never know you did not take the whole toilet out unless you ever have to replace the toilet. It is not a difficult thing to do.

Remove the toilet.
it's faster and easier than you think, and so's replacing it after you've finished painting.
1. Secure the inlet valve to the tank.
2. Disconnect the inlet line.
3. Flush the toilet.
4. Remove the two nuts holding the toilet securely to the floor.
5. Remove the toilet from its mount and wax seal.
Paint.
Then ...
1. Replace the toilet over the wax seal, carefully ...
2. Replace the nuts holding the toilet securely on the wax seal.
3. Reconnect the inlet hose to the tank.
4. Open the inlet valve.
You've done a perfect job of painting ... be proud of yourself. :)

Just turn off the water and remove the toilet. Then paint behind it and replace the toilet.

Unless it is some unusual type of toilet, you shouldn't have to remove the entire thing. You should have to remove nothing more than the tank to have enough room to reach the entire surface behind it and the bowl.
