Yes you do, for it to be legal.They need to sign a contract relinquishing their rights to the horse and that they recognize you as the owner and you are legally responsible for the horse.It needs to be signed and dated.
If you are looking to transfer on a registration form,again yes.Most orgs require it but some do not.Hope this helps.

It depends on the breed. *MOST* breeds yes.
If you have a TB with papers there is no real signature needed. If I buy a TB mare and I call to report "not bred" whatever status that year that is all the TB registry wants on older horses. There IS signatures needed for those on the track obviously.
