Supposedly a straight man; who dresses very well and is into a heighten state of physical grooming. In addition; he might be well cultured in the arts, music, news updates, and socially aware of various causes.

In my experience, it refers to a guy who is straight, but exhibits otherwise stereotypical "gay" behaviors - like knowing about and using more grooming products than what we (again, stereotypically) think the average straight guy would, being fashion conscious, etc. I heard it first a few years ago to describe a mutual friend who fit exactly that description, and I assume it's because in more urban settings, you get guys who devote more time to those kinds of endeavors - thus the "Metro" part of sexual.

A man that cares about his appearance...obsessed with getting their nails done (hand and toes) face clean shaven all the time, hair cut all the time, almost like a women...I live with one and he is great! I love a man that likes to look and smell good...by the way I do his nails polish and all (clear)
