A hold is awarded to a relief pitcher if he enters in a save situation, records at least one out, and leaves the game without having relinquished that lead.

"A hold is when a pitcher comes in either when the score is tied or when his team is behind and he doesn't allow the situation to get any worse."

No its not. Its when a pitcher comes in and hold their teams lead to set up the save for the closer. Thats why their called set up men. Heres the real definition: A relief pitcher is credited with a hold any time he enters a game in a save situation, records at least one out and leaves the game never having relinquished the lead. A pitcher cannot finish the game and receive credit for a hold, nor can he earn a hold and a save in the same game.
