There is only one geometric shape that can be safely used as a cover and never be able to fall through the Hole. No matter which way it is placed over it.. A round one.. That is why Man hole covers tend to be round.. no matter how it is laying over the hole it will nevr fall through. Make a hole with any other shape and the cover could be placed at an angle that would allow it to fall.
